What Helps You Determine Client Satisfaction on Creative Projects?

How do you gauge client satisfaction on your creative projects? Is it touch points throughout the creative project - one on one with the sponsor on the customer side? Is it a lessons learned session at the end of the project? Is it lessons learned type discussions through the project at key milestones or deliverables? Is it surveys or questionnaires? Obviously it's best to not wait until there is a major issue to find out your client is not happy with the project, so staying on top of this type of communication can be very important to client retention and future business. What indicators do you use to gauge the satisfaction of your project clients?
